Your First Appointment With Us:

Welcome to our practice! Your initial dental appointment with Dr. Biernath will include a complete examination of your mouth, including X-rays and periodontal probings. Treatment may start the same day, but complex cases will require a second appointment. New patient forms are available in English and Spanish and can be completed at the office or mailed to you beforehand. Please arrive 15 minutes early if you haven't filled out the forms. We look forward to meeting you!

Please provide us with the following when visiting us:our first time:

1. List of your current medications/supplements.

2. Current xrays from your previous dentist.

3. Contact info for your physicians.

4. Dental insurance details and p (if applicable).

5. IMPORTANT: Patients under 18 need a parent/guardian.

6. Notify us of any medical conditions or medications before treatment.

What about X-Rays?

If your previous dentist has taken X-rays in the last 6 months, you can request to have them forwarded or emailed to our office before your first appointment. If there's not enough time, please pick them up and bring them to your appointment. Any additional X-rays needed will be taken here to supplement the existing ones.
